Welcome to KidFit Sports Academy. As with any athletic program, training facility, league, or sports event, participants must acknowledge that there are inherent risks, including accidental and bodily injuries.
By accepting this waiver, you agree to assume all responsibility and liability for any accidents, and waive any claim for personal risk, bodily injury, sprains, ligament tears, emotional, or psychological damages. You confirm that, to your knowledge, the student has no limiting physical condition that would preclude participation in any KidFit Sports Academy LLC programs or events.
In case of emergency, the contact on file will be notified immediately. KidFit coaches will act accordingly, including calling for medical assistance if needed.
By accepting below, you take full responsibility for the student's health and well-being and acknowledge that no responsibility is assumed by KidFit Sports Academy LLC, its coaches, or program leaders.
